ACL graft failure Initial MRI from 3.5 years ago (first series), intact ACL graft with the tibial bioabsorbable interference screw seated proud at the midline intercondylar eminence. Follow-up (second series) shows the graft is completely torn and the screw fragment is located anterior to the lateral trochlea (and there is a new full thickness cartilage defect at the midline trochlea).
