Radial nerve sheath tumor
PNST, Schwannoma. Extending down to a very low bifurcation in the radial tunnel with supinator extensor denervation. DDx thrombosed vein and the anterior and posterior perineural vessels were the superficial radial n and PIN, with the PIN extrinsically compressed.
Avneesh Chhabra: On T2- arrow on neurovascular bundle. On T1 marks PIN is atrophic (thin). This is a segmental lesion as it will be impossible to remove without cutting nerve or grafting. Consider genetic studies and whole body MRI to exclude NF OR Schwannonatosis. If no PNST elsewhere or underlying immune deficiency or suspicion of lymphoma- get biopsy.
