Current MRI shows severe medial compartment cartilage loss (white arrow), 50% of the graft is torn and displaced into the notch (red arrow), there is complete meniscal body extrusion (blue arrow) with central meniscal flap in posterior medial notch. 6 mos ago there was acute ACL rupture and MM bucket handle tear, with no medial compartment cartilage loss or extrusion. In this case, the 50% tear of the graft is not so devastating as the rapid development of medial compartment OA. If the BHT was repaired, that failed. Now it is a large full thickness radial tear with meniscal extrusion, resulting in markedly increased femorotibial contact pressure and rapid development of OA.
