Axial T1w images show separate ext digitorum tendons in the 4th compartment at the level of Listers tubercle. At the proximal carpus the 4th ext compartment tendons are clumped together, appearing as if one huge tendon due to thickening and constriction of the extensor retinaculum. I have seen the clumped tendons misinterpreted clinically as a mass, often suspected to be a dorsal wrist ganglion. In this case there is intratendinous ganglion (not the location of the dorsal ganglion the referrer suspected). I am surprised the attached article indicates that hand intratendinous ganglia are rare as Ive seen several (though extensor is less rare than flexor ganglia). Apparently there is controversy regards to treatment. In their report they performed surgical excision. Can this be treated by US guided aspiration and fenestration of the retinaculum? Or is that risky? [ Article ]
