type II accessory navicular
In both the baseline (1 mo post injury) and current (6 mo post) images there is bright T2 signal in the synchondrosis, which may be minimally widened currently. There is increased subchondral BME at this time. In both, the PTT looks normal. Given the chronic and worsening symptoms after the initial injury, it might be that the accessory navicular is unstable. The attached article, in which 16 of 17 went to surgery, is surprising in that they found PTT pathology in 75%. In this, as in most cases, I find the PTT appears normal, despite the injury to the synchondrosis and surrounding marrow signal changes. [ Article ]
