intraarticular osseous body ( RID5367 )
Current MRI (slides 1,2) show a large anterior intraarticular ossific body with severe anterosuperior degenerative cartilage loss, which is new, and moderate superimposed effusion and synovitis. Prior MRI 4 years ago (slides 3-5) show localized ill defined intermediate soft tissue fullness within the anterior hip joint with a punctate dark focus, which was not reported. Instead, labral tear was reported and patient was treated with arthroscopic repair. In retrospect, I believe I would have reported localized synovitis, but I would not have imagined that this would have evolved into an intra-articular osteochondroma. Is the DJD secondary to this large mass? Hard to imagine its all that loose. Alternatively, this could be DJD with a huge intraarticular degenerative ossicle, but given the prior appearance with the localized soft tissue thickening at the same site, I favor an evolving intraarticular osteochondroma.
