Subscapularis tendon partial tear and anterior shoulder capsular sprain
Injury to the inferior subscapularis tendon and adjacent capsule. The patient refused an arthrogram. Subscap functions in late cocking phase in throwing by eccentrically contracting, applying a stabilizing anterior force to the glenohumeral joint, and halting external rotation. In acceleration phase, the subscap reaches maximum activity, producing the violent internal rotation of the humerus reaching forces as high as 185% of its maximum muscle test strength. Somewhere along this chain, the patient injured himself. I suggest it happened at the transition between late cocking and acceleration. [ Article ]
