transient osteoporosis ( RID5390 )
Referred for US guided MR arthrogram. Pre-injection US image shows synovial thickening (not vascular on Doppler). When I injected 1cc test of lidocaine, she was in extreme pain. 1 more cc saline + magnevist, crying pain. MD approved IA Kenalog and Naropin (relieved all pain). I am concerned there may be a small subchondral fracture, agree? I dont understand the synovial thickening. Is this subchondral fx simply? Do we agree TOH is truly a subchondral fracture?Could this be inflammatory (infectious?) arthropathy...but no changes at acetabulum. I told the MD to put her on crutches and draw serologies and WBC. Any other suggestions, opinions?
