biceps dislocation In order for the biceps to dislocate into the joint, the subscapularis has to tear from the lesser tuberosity. The biceps can dislocate from the groove and be superficial to the subscapularis but in my experience that is less common. This case shows nicely that the subscapularis inserts onto the lesser tuberosity but some superficial fibers extend to the greater tuberosity (some of them as the transverse ligament). In this case those fibers are still intact (at least some of them). This is a reason for a patient to fail the push off test for subscapularis integrity. In this case some strength will be preserved which can give a false negative test. That why you need MRI!
