Plantar plate tear Toe is nearly dislocated; the base of the 2nd toe is hinged on the impacted MT head. Diffuse, very high grade partial tearing, attenuation of the entire plantar plate, without avulsion at the insertion (which is unusual).
There is a small full thickness defect laterally where the ACL (accessory collateral ligament) blend with the lateral plantar plate, through which a ganglion extends into the plantar soft tissues of the 2nd intermetatarsal space and may well compress the digital nerve.
