PCL strain on ultrasound F29 with hyperextension injury to knee 2.5 months ago. Unexplained persisting posterior knee pain, now starting to resolve. Initial MRI noted hyperextension pattern of tibiofemoral bone bruising anteriorly but reported PCL as intact. At US exam, site of pain and tenderness correlated with an abnormal PCL which showed hypoechoic swelling (arrowheads) and intrasubstance tear (arrow). In retrospect, I think the MRI showed abnormal periligamentous signal as the only clue.
