posteromedial bundle of posterior cruciate ligament ( RID2786 )
There is intermediate signal soft tissue thickening along the medial aspect of the PCL with what appears to be a tear near the tibial insertion of the posteromedial bundle in the Sag images. The larger anterolateral bundle is intact, with minimal increased intrasubstance signal in the vertical segment. This is an isolated injury, so it will certainly be treated conservatively. I know there has been controversy over soft tissue thickening in this location, with reports of synovial thickening/scarring with intact PCL. Note that the opposite knee does not look like this (image 5). Even though this should heal with stability, I wonder if this might be the cause of scarring we sometimes see in this location and if it might not be a source of chronic pain and limitation.
