Referred by sports orthopedist requesting evaluation of suspected rotator cuff tear. Not the most likely clinical diagnosis, given the recent acute atraumatic onset of pain and LROM in someone with pre-existing quiescent insertional supraspinatus calcific tendinosis (white arrow) and infraspinatus peritendinitis (red arrow) with overlying subdeltoid calcific bursitis (slide 3). Now the supraspinatus deposit is much larger and has eroded into bone with geographic marrow edema, as well as overlying subdeltoid bursal inflammation.
