volar plate tear It appears that there is full thickness midsubstance rupture of the lateral portion of the index MCP volar plate (white arrows); the medial portion looks thick and frayed, but I dont see a defect. I have never seen a midsubstance volar plate rupture and Im not sure if it requires repair, as I suspect the fibrocartilage has poor healing potential. In addition, there is distortion with soft tissue thickening and both dark and slightly bright signal on PDFS along the volar radial nvb. Would you recommend US to better define the extent of the volar plate tear? Im inclined to recommend US to evaluate possible injury to the neurovascular bundle. Have you ever seen a midsubstance volar plate rupture? Do you know if its treated surgically? [ Article ]
