plantar plate of metatarsophalangeal joint ( RID3134 )
Not the typical demographic....young male...but he has HAV and a slightly long 2nd MT. His 2nd MTPJ plantar plate tear is limited to the lateral insertion (midline capsular recess 2.5mm). There is fraying and tiny partial tear of the lateral capsule. Reactive BME at the plantar lateral base of the 2nd toe proximal phalanx. Plantar and lateral pericapsular soft tissue thickening and edema. Long axis T1 shows smooth flattening of the 2nd MT head...I see this alot with plate tear, sometimes acute/subacute Freibergs (makes sense, with HAV and long 2nd MT). Its really essential to properly plan the Sag plane to maximize in-plane imaging of the plantar plate---cant really assess length of the midline recess out of plane (that is how to determine if the tear is extending to or through midline).
