I think these intra-tendinous, peritendinous / subdeltoid bursal tiny calcifications are easy to miss on MRI (US would be better, might be difficult or impossible to see on XR). I doubt these would be visible at 0.3T. The main clue that this is not non-calcific subscapularis tendinosis or partial intra-substance cuff tear is that the mid-inferior (rather than cranial) insertion of the subscapularis tendon is affected. It appears that tiny calcifications have been extruded from the supraspinatus, with superficial fraying and mildly bright T2 signal at its insertion and tiny calcifcations in the overlying, inflamed subdeltoid bursa.
