There is diffuse thinning of the suprapinatus tendon with full thickness tear posteriorly; the myotendinous jxn is retracted but there is little supraspinatus atrophy. There is a contiguous partial articular surface delaminating tear of the IST extending to the myotendinous jxn, best demonstrated in the BLO sequence. Axial images show partial articular surface delaminating tear of the subscap (note the extreme ER of the arm) with medial subluxation of the LHBT as there is a type 3 Bennett biceps pulley lesion. There is a low lying os acromiale with a subacromial enthesophyte and degen changes at the synchondrosis. [ Article ]
