I dont often see isolated subscapularis avulsion. There is supraspinatus tendinosis, but no supraspinatus tear. As I find the incidence of pathology seems to be lower for a history of work related injury, this case was a refreshing surprise. The absence of muscle atrophy on T1w argues that it is an acute injury. The biceps is located though I imagine it might be unstable: there is a tear of the transverse humeral ligament and the pulley must be torn. [ Article ]
