If you see a bright T2 signal defect at the plantar plate (PP) insertion, youve got a tear. These tears can be very subtle or occult on MRI. In this case, its neither. The tear extends from medial to lateral. In the midsagittal image (2) the distance from the base of the prox phalanx to the retracted PP is 5.2 mm; in this location there is a normal, discrete bright T2 signal zone that does not exceed 2.5mm. Note the mild extension of the 2nd toe and the MPJ and the submetatarsal and intermetatarsal bursal inflammation.
