posterior root of medial meniscus ( RID2777 )
I read the initial MR as an intact medial meniscus and posterior root. 19 days later there was a large radial tear at the junction b/t the meniscal root and posterior horn, with med meniscal body extrusion. Mild cartilage thinning initially, possibly slightly increased on f/u. I am told that the meniscal root fell apart on probing at scope and the surgeon decided to repair it as a temporizing measure prior to TKR. Clear progressive cartilage loss on XR pre and post: dont know if that happened prior to or after repair. I am surprised a root repair was done in a 53 yr old with med compartment cartilage loss. I am also surprised that a repair was contemplated when the lesion extended so distal to the root insertion, so that healthy tissue would have needed to be pulled out of position to anchor at the root (and only the 1cm segment from the anchor is well vascularized and has good healing potential). Even in retrospect I dont see the meniscal tear or root lesion on the 1st MRI. Your thoughts?
