talar and calcaneal stress fractures
There are a variety of arthroeresis implants utilized to control pathologic subtalar pronation in flexible flatfoot deformity. It is primarily used in skeletally immature individuals but has been used with success in (other) adults. Complications can be related to malpositioning of the implant, reactive synovitis (with bioabsorbable implants) and fracture, as in this case. As the posterior talar fracture is healing, there is a new subchondral fracture of the anterior calcaneus and a new cortical avulsion fracture at the dorsal talar insertion of the dorsal talonavicular ligament. I dont know enough about these devices to know why this patient is crumbling. At a certain point I suppose it may be feasible to convert to arthrodesis.
