calcific periarthritis ( RID3597 )
As per usual, there is a CAM lesion and anterior labral tear. My guess is that symptoms related to FAI and labral tear would not present so acutely. There is localized soft tissue inflammation posterior to the subtrochanteric femur which I believe is most closely related to the insertion of the Adductor Brevis (rather than the Gluteus Maximus). Amidst the bright T2 signal in the intermuscular fat planes (just anteior to the sciatic nerve, by the way), there is an amorphous focus of dark signal which I presume is calcification. I believe that would explain acute, recent onset of pain. Ive not previously seen this related to the Adductor insertion, but have seen several in the Glut Max insertion. Thoughts?
