On ultrasound, the median nerve at proximal carpal tunnel level was slightly swollen (transverse cross-sectional area 11 mm2), but an interesting contributing feature to increased carpal tunnel pressure in this case was something that you will never see using current MRI protocols ... Marked lumbrical intrusion on flexing the fingers to make a fist, such that the proximal margin of 2nd lumbrical muscle could be seen to reach the level of distal radius !
