Discussion

Normal variant anatomy of the PTT that is not described among the reported normal variants (see ref). There is no insertion onto the navicular. The major slip is tendinotic and partially torn, coursing plantar to the navicular to insert onto the plantar base of the med cuneiform. Note (slide 2) that the normal tib anterior tendon inserts more distally onto the plantar medial head of the med cuneiform. There is a smaller accessory slip of the PTT inserting onto the plantar base of the lateral cuneiform. Just crazy. Reference article.
