History

49F felt pop 3 weeks ago with persistent pain. Ultrasound in orthopedic office reported as peroneus tenosynovitis, otherwise normal.

Discussion

Peroneus longus tears at the midfoot are relatively uncommon and are often attributed to friction with a large lateral calcaneal peroneal tubercle, or can be associated with an os peroneus. This patient has neither. I am surprised that the thickened retracted proximal remnant of the peroneus longus was not appreciated by the examining orthopedist on ultrasound.
