Isolated avulsion of the lesser tuberosity is a rare injury and suggests subscapularis avulsion. In this case, the avulsion is chronic: the mature ossicle is embedded in the retracted subscapularis tendon and there is severe muscle atrophy. BME in the anterior humeral head reflects acute injury 3 days prior to MRI. Note the posterior glenoid labral avulsion. Joint fluid courses through the anterior capsular/subscapularis defect.
